Primary care physicians are essential to our patients’ overall healthcare experience, which is why we formed the Pennock Physician Network (PPN) in 2011. The PPN is an integrated team of 11 primary care and specialty physicians, along with their nurse practitioners, who provide high quality medical care. This new, comprehensive team of healthcare professionals work together to deliver continuity of care throughout the entire Pennock Health System.


Just as every patient is unique, so is every healthcare professional. Each PPN provider is committed to comprehensive, quality care as well as compassionate and attentive treatment.

 

This integrated network of board certified and board eligible medical professionals has been trained at medical institutions across the United States and provide superior healthcare services to you as part of the Pennock Health Services family. Our physicians are close to home and strategically located throughout Barry, Ionia and Kent Counties, so outpatients can concentrate on living their lives, not worrying about access to health care.

Patient-Centered Medical Home or PCMH is an established model for care that has been refined over the past few years, emphasizing more personalized, coordinated, effective and efficient care for patients. Pennock believes that nothing should stand between a patient and his/her good health.


PCMH strengthens this physician-patient relationship with the goal to move from episodic care to care that is more coordinated, strategic and integrated. The physician and the patient look at the patient’s health history and current health; then together, they develop a comprehensive, on going personal care plan.


This health relationship does not stop with the primary care physician or a certain period of time in your life, but is meant to continue on through all stages of life including preventive services, chronic, and acute care. A patient’s personal needs are attended to through every stage of life. Your care and the accountability for that care are passed from one team member to the next without any loss of quality or continuity. The desire is a positive health outcome with an enhanced total experience for everyone.
