Clinical Informatics Specialist
Department: Information Technology
Status: Full-time, salaried
Schedule: Generally Monday-Friday, first shift
Requirements: Must be RN, BSN preferred. Minimum of three years of experience caring for patients in a hospital setting. Experience with EMR required and prefer knowledge of EMR implementation.
Responsibilities: Acts as liaison between Hospital's clinical areas and IT department. This includes primary responsibility of determining the needs and requirements for clinical applications in acute and ambulatory healthcare settings. Will be instrumental in the evaluation, selection, and implementation of clinical systems (i.e. Nursing, ED, CPOR, Homecare). Will work with staff and physicians to optimize the usage of clinical systems through education and training, quality outcomes, and providing support in accordance with organizational goals.
Health Coach
Department: Family Medicine Offices
Status: Full-time, 80 hours per pay
Schedule: First shift, Monday-Friday
Requirements: Must be RN with current Michigan license. At least two years in an office practice setting is required. Experience with JCAHO, PCMH and EMR, preferably eClinicalworks, a plus. Must be knowledgeable in the area of PIP/PRP/PGIP measures. Strong teamwork, customer service, and communication skills are essential. Must possess valid Michigan drivers license as travel is required daily (mileage reimbursed).
Responsibilities: Responsible for the coordination of Patient-Centered Medical Home (PCMH), Quality Incentive Measures, Health Coordination for all five Family Medicine Offices. Works collaboratively with office managers and providers to ensure successful implementation of PCMH, leading to PCMH designation for each office.
Home Health Aide-Homecare and Hospice
Department: Homecare/Hospice
Status: Casual part-time, as needed
Schedule: Weekdays, first shift and occasional weekends for Hospice patient care.
Requirements: High school diploma, successful completion of a nurse aide class and one or more years of experience as a home health aide or CNA in Homecare, Hospice, Hospital, or long term care setting. Certification in Homecare/Hospice will be required; may obtain afer hire if experienced as a home health aide.
Responsibilities: Assists in activities of daily living in the patient's home. Must be able to lift up to 50 lbs, stand for long periods of time, bend/reach without restriction. Must also possess valid Michigan driver's license and have reliable transportation (mileage is reimbursed).
LPN or Certified Medical Assistant
Department: Gun Lake Family Medicine
Status: Full-time, 80 hours per pay
Schedule: Four weekdays, typically 7:30 am-4:30 pm
Requirements: Licensed LPN or Certified Medical Assistant required, one or more years of physician office experience required if CMA. A high energy team player with strong customer service skills and multi-tasking abilities is needed. Ability to walk/stand for long periods is also required.
Responsibilities: Assists provider in the clinical treatment of patients. Provides direct and indirect patient care in an ambulatory care setting. Addresses the comprehensive needs of our patients, delivers care to a diverse patient population. Will initiate appropriate clinical interventions and education in conjunction with the Provider's comprehensive plan of care. Reports to Office Manager, performs other duties related to a busy family practice as assigned.
LPN or Certified Medical Assistant-casual part-time

Requirements: Licensed LPN or Certified Medical Assistant required, one or more years of physician office experience required if CMA. A high energy team player with strong customer service skills and multi-tasking abilities is needed. Ability to walk/stand for long periods is also required. Requires valid Michigan drivers license and reliable transportation for travel to outlying offices.
Responsibilities: Assists provider in the clinical treatment of patients. Responsible for all clinical and clerical duties associated with a busy family practice such as prescriptions, EMR, cleaning, etc.
Registered Nurse - Emergency Department

Status: Part-time, 48 hours per pay
Schedule: 11:30 am-midnight and every third weekend and two of six holidays per year
Requirements: Must be RN with current Michigan license, ACLS required within six months, and TNCC required within one year. Previous ED experience is required. BSN preferred.
Responsibilities: Staff RN. Accountability for decision making relative to the nursing care of patients. Apply hospital policies and nursing department standards of care and practice in the rendering of physical and supportive care, health teaching to patients and families. Must possess excellent customer service skills.

Schedule: Variable hours and weekend rotation
Requirements: Must be RN with current Michigan license and OB certification preferred, BSN also preferred. OB experience and at least one year of Med/Surg experience are required.
Responsibilities: Labor and delivery, antepartum and postpartum care, nursery care, stabilization of mothers and babies for transfer.
